Belt Parkway Eastbound to Exit 20-Rockaway Blvd.
Proceed along South Conduit (Service Rd) to Rockaway Blvd. (2nd traffic light)
Make left onto Rockaway Blvd. (under overpass) and stay in center lane.
Proceed north on Rockaway Blvd. 4 blocks to 134th Avenue (1st traffic light)
Make left - 1 story bldg. on northwest corner of 134th Ave & Rockaway Blvd.

From Queens/NYC:
Van Wyck Expressway toward Kennedy Airport.
Either Exit at Rockaway Blvd and make left at light (Rockaway Blvd)
Continue past 150th St. (Baisley Park) and make right 4 blocks at 134th Ave 
(2 traffic lights) - just before Gulf Station.

Or,

Follow signs towards Belt Parkway-East. Don't get on parkway, get on South
Conduit Ave and continue until Rockaway Blvd (2nd traffic light). 
Make left onto Rockaway Blvd - stay in center lane. 
Proceed 4 blocks (1st traffic light) to 134th Ave. Make left.

From Long Island:
Belt Parkway Westbound to Exit 21A-Rockaway Blvd.
Move to right and make right at Rockaway Blvd. (1st traffic light)
Proceed 4 blocks to 134th Avenue (1st traffic light)
Make left-one story bldg. on northwest corner of 134th Ave. & Rockaway Blvd.
